A: Sometimes this can be a physical problem, and sometimes this can be a driver issue. Have you tried updating/reinstalling your mouse drivers, and in addition have you tried cleaning out your mouse? Don't use liquids, a simple tissue or cloth should do the trick - and on optical mice you can use cleaning fluids.
